APPLICATION FOR CHANGE OF NAME AND ADDRESS LISTING
1. The request
for registration of the change of name and / or address of the owner of the
registered trademark shall be filed in writing in Indonesian language by the
applicant by way of typed copies of two (2)
2. The
application contains clear about:
• Brand name
and registration number of the trademark being applied for registration of the
change of name and/or address;
• Name and
address of the previous owner; and
• Name and
address of the new owner.
3. The applicant must attach:
a. Evidence of
change of name and/or address,
b. Special
power of attorney if the request for registration of the change of name and/or
address filed by the authority;
c. An official copies
of the certificate of incorporation legal entity or its copies certified by a
notary public, if the applicant legal entity.
d. Copy of
certificate of registration of trademark being applied for change of name and/or
address
e. A copy of
the applicant's identity card, and
f. Proof of
payment of application fee.
APPLICATION FOR REGISTRATION CANCELLATION MARKS
REGISTERED
1. Application
for cancellation of the registration of a registered mark shall be filed in
writing in Indonesian language by the applicant by way of typed copies of two
(2);
2. Applicant
must attach:
a. Court
decisions that have permanent legal strength or a certified copy of the
judgment by the court
b. Special
power of attorney, if the petition is filed by the authority;
